Job overview

The Clinical Assessment Registered Nurse (RN) – Home Health is an onsite role in Glen Allen, VA, serving Richmond and surrounding Virginia communities. Compensation is $130 per assessment/SOC visit, $70 per follow-up visit, $90 per recertification, and $100 per discharge; mileage reimbursement is $0.50 per mile. Vision Healthcare’s assessment nurses help determine patient needs and support care in patients’ homes. The RN conducts in-home assessments and visits, develops and updates individualized care plans, and coordinates with physicians and the interdisciplinary team. The role contributes to timely, appropriate services through clinical judgment, patient and caregiver education, and accurate documentation.

What you'll do

• Conduct comprehensive in-home nursing assessments and admissions
• perform Start of Care, follow-up, reassessment, recertification, supervisory, and discharge visits
• assess patient condition, functional status, medications, risks, and care needs
• develop and update individualized care plans
• communicate changes to physicians and the clinical team
• educate patients and caregivers
• coordinate care
• document in the agency EMR
• maintain regulatory and agency compliance.
