What you'll do

• Support individual service plans and monitor objectives
• train and assist residents with activities of daily living
• accompany residents on outings and travel training
• provide counseling, behavioral intervention, and crisis management
• assist with residence operations and medication management
• document daily progress
• provide transportation as needed
• advocate for residents and coordinate with service providers.
