What you'll do

  • Develop individualized care plans with the CTI team
  • conduct physical, mental, and behavioral health assessments
  • coordinate appointments, prescriptions, referrals, and community resources
  • educate clients on chronic conditions, medication adherence, and preventive care
  • provide clinical support during crises
  • collaborate with social workers and case managers
  • support discharge planning
  • document encounters and progress
  • and advocate for clients’ healthcare needs.

Why this role

A bachelor’s degree in nursing is required; an associate’s degree may be considered with additional experience. Active New York State licensure is required. The role includes community travel and flexible hours, including some evenings and/or weekends.
