Wellness Nurse

The Wellness Nurse role is onsite in Birmingham, AL, with a 6:00 a.m.–2:00 p.m. schedule, Monday through Friday and every other weekend. Compensation is not specified. The nurse supports resident wellness at The Bungalows at Riverchase and reports to the Wellness Director. The Wellness Nurse monitors residents’ health and wellbeing, supports resident care, and helps maintain clinical quality. The role contributes to appropriate care by identifying changing needs, documenting follow-through, and coordinating with the Wellness Director and resident support services.

What you'll do

• Administer medication care
• communicate with resident support services and families
• identify resident needs through assessments and Personalized Service Plans
• document follow-through when clinical needs change
• assist with monthly wellness visits and service plans
• report medication or service-level changes
• obtain monthly weights and vital signs
• provide direct resident care as needed.
