What you'll do

• Manage a caseload of patients with multiple chronic conditions
• develop personalized care plans
• assess medical, social, and lifestyle factors
• monitor progress and adjust plans
• educate patients and families
• use telehealth and remote monitoring
• coordinate with Care Navigators, providers, and interdisciplinary team members
• support medication reconciliation and appointment coordination
• help reduce readmissions and emergency visits
• maintain HIPAA- and CMS-compliant EHR documentation
• and track outcomes and quality measures.
