What you'll do
- Complete initial and ongoing in-home nursing assessments of physical, cognitive, psychosocial, and home-safety needs.
- Create and update individualized care plans and ensure caregivers understand and follow them.
- Manage client caseloads, conduct supervisory visits and competency checks, coordinate with families, physicians, hospitals, and community partners, make referrals when needed, and maintain accurate records in accordance with regulations, infection control, and HIPAA.
- Provide on-call support for client and caregiver concerns.

Why this role
Monday–Friday day shifts, 8 a.m.–5 p.m., with flexible scheduling and occasional holidays. Requires an active, unencumbered Colorado RN license, at least one year of home care or home health nursing experience, current BLS/CPR, and a valid driver’s license with reliable transportation.
