What you'll do

• Provide case management for clients with moderate to high clinical and psychosocial needs
• coordinate discharge planning and transitions
• assess needs and barriers
• connect clients with housing, employment, transportation, insurance, benefits, healthcare, and recovery supports
• coordinate referrals and follow up with providers
• document services in the EHR
• participate in treatment team and quality improvement activities
• maintain confidentiality and professional boundaries.
