What you'll do
- Provide psychoeducation and assist with crisis and treatment interventions
- support relapse-prevention and disease-management strategies

- provide intensive case management and referrals
- monitor and document outcomes
- assist with housing searches and housing stability
- coordinate services and transitions
- participate in the 24/7/365 first-responder on-call system
- maintain confidentiality and required records.

Why this role
