What you'll do

• Conduct hospice admissions and administer nursing care in patients’ residences
• assess physical, psychosocial, and environmental needs using nursing assessment and LCD criteria
• develop and document plans of care
• provide nursing care using infection-control measures
• coordinate with the interdisciplinary group, patients, families, referring agencies, and physicians
• submit required paperwork on time
• participate in hospice in-service programs and the QAPI program
• perform assigned on-call responsibilities and provide on-call services to patients and families
• travel to patient homes and office or support center locations as needed.
