Client Care Coordinator

Job overview

What you'll do

• Enter new clients, billing, and service agreement dates in AxisCare
• process referrals and document client information
• coordinate with the scheduler, HR, and Director of Operations on care needs and conflicts
• follow up with new clients and conduct satisfaction surveys
• manage scheduling changes and notify clients
• coordinate service plans
• communicate with providers, families, and clients
• monitor contract compliance and service documentation
• report over, under, and missed visits and document call-offs
• build community and healthcare-provider relationships
• represent the agency at community events
• answer calls and greet visitors
• maintain confidentiality and HIPAA compliance
• provide backup caregiver support when needed
• conduct client outreach and visits as described
• monitor care notes
• perform other duties as directed.
