What you'll do

• Assist residents with activities of daily living according to service plans
• support individual needs, preferences, and dignity
• observe and communicate changes in condition
• maintain documentation and follow care, safety, HIPAA, and regulatory requirements
• collaborate with Care Partners, clinical leaders, families, and community team members
• assist with meals and dining-area upkeep.
