What you'll do

  • Coordinate with primary care and community providers
  • visit clients in hospital and community settings

  • provide disease-management education and coaching
  • reassess clients and revise plans
  • arrange services and advocate for access
  • use EHR reports to identify and enroll eligible patients
  • and support team training and quality improvement.
