Treatment Coordinator

Job overview

The Treatment Coordinator role is hybrid or fully onsite in Baltimore, MD, with a Monday–Friday schedule of 8:00 AM–5:00 PM EST. Compensation is $25–$45 per hour or $50,000–$167,560 per year fee-for-service, depending on work setup and engagement type, credentials, and experience. The role supports clients receiving outpatient mental health services at Community Wellness Outpatient Mental Health Center. The coordinator manages an assigned caseload, provides weekly clinical support, and coordinates treatment and referrals so services remain aligned with client needs and program standards. As Treatment Team Lead for assigned clients, the coordinator also supports team members and helps address barriers to access, communication, documentation, and service delivery.

What you'll do

• Provide weekly clinical support to assigned clients
• assist with assessments, treatment planning, and progress monitoring
• coordinate care with therapists, psychiatric providers, PRP staff, case managers, and community providers
• track referrals
• support client engagement, care transitions, and continuity of care
• maintain timely electronic medical record documentation that meets agency, payer, and regulatory standards
• review treatment activity against client goals and needs
• participate in supervision, audits, training, and quality improvement
• serve as Treatment Team Lead and coach treatment team members.
