The Community Health Worker role is onsite in South Milwaukee, WI; compensation is not specified. The Muslim Community & Health Center is preparing to open an additional location to expand access to its services and reach more community members. As part of the interdisciplinary care team, the worker helps patients navigate healthcare and community resources, coordinates follow-up and referrals, and provides basic health education and outreach. The role helps address barriers affecting access to care while communicating patient needs to clinical staff and community partners within the position’s scope.

What you'll do

• Help patients navigate healthcare services, appointments, referrals, and follow-up
• identify barriers to care and connect patients with medical, behavioral health, dental, social service, and community resources
• provide basic health education and participate in outreach activities
• document interactions and referrals
• communicate relevant needs to the care team
• maintain confidentiality and professional boundaries.
