What you'll do

• Manage up to 15 cases
• conduct intake, assessments, and initial service planning
• provide housing referrals and support
• screen program eligibility
• coordinate with VA personnel and other providers
• assist with education, training, employment, and mainstream needs
• conduct outreach and home visits
• provide crisis-oriented counseling and interventions
• maintain participant records, databases, referrals, and HMIS data
• attend team meetings and training
• maintain confidentiality.
