Nurse Case Manager, Medicaid New Choices Waiver

What you'll do

• Complete medication management reviews
• assess clients’ health conditions and identify needed supports
• evaluate and coordinate services
• review care and medications regularly and after health-status changes
• complete and update client documentation
• communicate with social work case managers, facility staff, and service providers
• attend staff meetings and training
• and travel to visit clients each month.
