What you'll do

• Receive, review, and process referrals
• collect accurate intake information and coordinate handoffs to clinical teams
• handle inbound calls and inquiries
• gather and securely maintain client health information in the EMR
• share referral-partner updates with Sales
• assist operational teams with care coordination and service improvement.
