What you'll do

• Assess clients’ nursing needs, conduct initial face-to-face home assessments, provide nursing services according to service plans, supervise CNAs and home health aides, complete clinical documentation, perform supervisory visits, evaluate and revise plans of care in consultation with physicians, provide hands-on care and case management, educate clients and families, assess and manage pain, and report changes in condition to physicians.
