Home Care Physical Therapist - Per Diem

The Home Care Physical Therapist - Per Diem role is an onsite position based in Manhasset, NY. Compensation is $70-$90 per visit, and this role supports the multidisciplinary team at Metro Physical & Aquatic Therapy in delivering high-quality care to patients in their homes. This role exists to provide essential physical therapy services that help patients maintain mobility, independence, and quality of life. The therapist evaluates patient needs, develops individualized treatment plans, and implements therapy programs to restore function while ensuring compliance with agency standards.

What you'll do

• Accept and treat patients within designated geographic area
• Evaluate patients' needs for physical therapy in the home setting and develop individualized treatment plans
• Develop a plan of care and treatment goals in consultation with the physician, patient, and family
• Plan, prepare and implement customized therapy programs to restore function and promote independence
• Instruct family members, PTAs, Home Health Aides, and caregivers on appropriate therapy techniques
• Provide timely and accurate documentation for each visit
• Communicate effectively with patients, families, and interdisciplinary team members
• Deliver care in compliance with agency policies and professional practice standards.
