The Home Health Registered Nurse (RN), Weekend Coverage role is an onsite field position serving the south part of Greater Houston, including Sugar Land, Missouri City, Pearland, Webster, Pasadena, Texas City, and surrounding areas. Compensation is $100–$150 per hour. All About Home Care is a Houston-based home health agency serving Houston and surrounding communities since 2004. The nurse evaluates patients’ needs and provides skilled nursing care under each patient’s plan of care. The role documents visits, coordinates with physicians and other clinicians, educates patients and families, and supports continuity of care through weekend coverage.
What you'll do
- Provide services according to the plan of care
- conduct initial evaluations and regularly reevaluate nursing needs
- provide services requiring specialized nursing skills
- initiate preventive and rehabilitative nursing
- prepare clinical and progress notes
- inform personnel of changes in patient condition
- counsel patients and families
- process orders and notify physicians of needs or changes
- complete certification and recertification orders and discharge summaries
- refer patients to other clinicians when appropriate
- supervise and teach nursing personnel
- participate in patient care conferences and assigned peer review and performance improvement.
