What you'll do

• Review clinical referral packages for skilled nursing placement
• conduct bedside assessments at assigned hospitals
• manage referrals through facility handoff
• coordinate with facility interdisciplinary teams
• build relationships with hospital personnel
• identify referral opportunities and market trends
• conduct educational meetings and presentations
• maintain referral, conversion, market, and outreach records in the company CRM.
