Registered Nurse (RN) Case Manager

The Registered Nurse (RN) Case Manager role is onsite in Salem, OR, with a Monday–Friday, 8:00 AM–5:00 PM schedule. Pay is $52–$62 per hour. The position joins a growing home health clinical team serving Salem and surrounding communities. The case manager assesses patients, develops and updates individualized care plans, and coordinates care with physicians and interdisciplinary teams. The role supports continuity of home health care through patient monitoring, ordered treatments, and education for patients and families.

What you'll do

• Complete patient assessments and develop individualized plans of care
• monitor patient progress and revise care plans as needed
• administer medications and treatments according to physician orders
• educate patients and families about health conditions and care plans
• coordinate care with physicians and interdisciplinary healthcare teams.
