Refugee Foster Care Case Manager

What you'll do

• Manage an assigned caseload of refugee youth in foster care
• conduct monthly in-home visits and assess progress
• develop and implement individualized service plans
• prepare court reports and attend required hearings
• document case activity and maintain compliant files
• submit required federal reports
• provide crisis management and conflict resolution support
• coordinate referrals and services with youth, foster families, staff, and community partners
• participate in weekly staff and monthly case management meetings.

Why this role
