Community Health Worker

Job overview

The Community Health Worker role is onsite in Johnson City, NY, with pay of $19 per hour. Mothers & Babies Perinatal Network’s Perinatal and Infant Community Health Collaborative supports women and families in high-need communities. The role connects community members with health information, insurance, healthcare, and supportive services. Through outreach, client-centered assessment, home visits when appropriate, referrals, education, and follow-up, the worker helps clients maintain access to care and strengthen health-promoting behaviors.

What you'll do

• Conduct neighborhood outreach
• identify client and family needs and complete risk assessments
• coordinate high-risk cases with the Program Manager
• provide home visits for eligible clients
• assist with insurance, healthcare, family planning, and community-service access
• share health information
• link families to resources
• arrange group education
• enter client data and assist with reports.
