What you'll do

• Coordinate daily and weekly schedules for personal care aides, caregivers, and home health clinicians
• match staff with patients based on availability, location, skills, and needs
• communicate schedule changes to patients, families, caregivers, clinicians, and office staff
• respond to call-outs, late arrivals, and staffing emergencies
• arrange replacement coverage
• monitor upcoming visits and resolve scheduling gaps
• assist with new-patient and start-of-care scheduling
• maintain schedules, staffing records, and communication notes
• escalate urgent patient-care concerns and unresolved staffing issues
• protect confidentiality and follow HIPAA and agency policies.
