What you'll do

• Provide outpatient primary care within Family Medicine or Med-Peds
• diagnose and manage acute and chronic conditions
• conduct wellness visits, preventive screenings, and physical examinations
• develop individualized treatment plans
• educate patients
• coordinate referrals and follow-up
• maintain EHR documentation
• collaborate with care managers, behavioral health professionals, advanced practice providers, and specialty teams.

Why this role

Weekday outpatient schedule; openings across four Chicago-area locations. Spanish or Polish fluency is preferred but not required. Board certified or board eligible in Family Medicine or Internal Medicine-Pediatrics; active Illinois medical license or ability to obtain one before starting.
