What you'll do

• Conduct patient outreach and assess behavioral health needs
• determine program eligibility and schedule intake appointments
• coordinate referrals with primary care and other providers
• participate in multidisciplinary meetings
• maintain referral and emergency logs and EHR documentation
• support crisis response
• coordinate higher-level care and community services
• follow up on missed appointments
• assist with scheduling and pharmacy requests as authorized.
