Compensation is $75,000–$80,000 per year. The role supports the Weinberg Center for Balanced Living, an older adult center serving adults age 60 and older through the Educational Alliance. The social worker provides case management and supportive services that help older adults address complex aging-related needs and age in place with dignity. The role contributes to holistic wellness through assessments, care planning, referrals, advocacy, group workshops, and coordination with internal and external partners.
What you'll do
- Maintain a caseload of older adult clients
- conduct intakes and comprehensive assessments
- develop care plans and recommend interventions
- assist with housing and benefits applications and renewals
- make referrals and advocate for services
- provide supportive contact
- facilitate or co-facilitate workshops
- maintain required electronic records and documentation
- coordinate with agencies and internal partners
- provide interpretation or translation access
- travel between program sites.
