Health Navigator

The Health Navigator is an onsite, full-time role based in Beckley, WV, working inside a partner clinic. Compensation is not specified. Main Street Health partners with rural primary care doctors to support relationship-driven, value-based care in rural communities. The navigator builds trusted relationships with patients and providers and handles patient-facing, administrative, and logistical tasks that help clinicians focus on medical care. By coordinating appointments, procedures, follow-up, and access to resources, the role helps patients navigate care and address gaps in their health needs.

What you'll do

• Build relationships with patients
• meet patients during check-in and enter data into Main Street’s software
• call patients to schedule visits or remind them to pick up medication
• order and schedule procedures, tests, and screenings
• track gaps in care
• document interactions and communicate with providers through electronic health records
• educate patients about healthcare options, insurance benefits, and common conditions
• connect patients with community resources
• and coordinate follow-up after hospital discharge. Schedule is Monday–Friday, 8 a.m.–5 p.m., with a one-hour lunch
• requires an active, unencumbered driver’s license and a GED or high school diploma.
