The Social Worker role is onsite in Methuen, MA. Compensation is $25–$50 per hour. The position supports residents and families at Cedar View Rehabilitation and Healthcare Center, a 106-bed sub-acute and long-term care facility. The social worker assesses psychosocial needs, develops individualized care plans, and coordinates services to support residents’ emotional and social well-being and continuity of care. The role collaborates with residents, families, case managers, and interdisciplinary clinical teams to promote well-being and care outcomes.

What you'll do

• Evaluate current, new, and prospective residents’ psychosocial needs, strengths, and limitations
• address concerns, goals, and emotional needs
• communicate with residents, families, and case managers
• build supportive relationships
• coordinate care with interdisciplinary teams
• develop, implement, and update psychosocial care plans
• participate in care planning meetings
• provide counseling, emotional support, and resource referrals
• comply with facility policies, regulations, and documentation standards.

Why this role
