What you'll do

• Develop the assigned territory through in-person relationship building
• establish and maintain referral partnerships with physicians, hospitals, skilled nursing facilities, assisted living communities, and other providers
• generate business through outreach, cold calling, and networking
• meet referral and performance targets
• serve as primary contact for referral partners
• collaborate with clinical and operational teams on patient transitions
• track activity and pipeline in CRM systems
• represent the organization at community events and professional engagements.
