Health Access Navigator

Job overview

The Health Access Navigator role is an onsite, full-time position in San Francisco, CA. Compensation is $30–$31.50 per hour. The role supports GLIDE’s Health Access Programs, which connect community members with health services and support. The navigator provides harm reduction supplies, health education, outreach, and service referrals for people with substance use and co-occurring disorders. The role helps reduce barriers to care and supports transitions to longer-term services through client-centered engagement and community partnerships.

What you'll do

• Provide harm reduction supplies through the syringe access desk and community outreach
• promote and recruit clients for syringe access, HIV/Hep C, health, social, recovery, and treatment services
• assist with data collection and monthly reports
• provide group education and presentations
• support overdose prevention education and Narcan distribution
• coordinate outreach
• document service contacts and client progress
• maintain confidentiality and professional boundaries.
