What you'll do

• Accompany individuals to job sites, observe performance, and provide work training and job-retention assistance
• help gather participant information about employment history, education, skills, abilities, and barriers
• act as an advocate and liaison with site personnel
• support daily living activities, personal care, meal preparation, and medication management
• plan activities and outings
• collaborate on individualized care plans
• assist with routines such as cooking, cleaning, shopping, and medical appointments
• follow HIPAA standards.
