What you'll do

• Provide professional nursing care

• monitor and follow up with clients
• provide on-call telephone services
• assess clients and identify discharge-planning needs
• document encounters
• administer prescribed medications and support medication management
• provide wound and urinary care
• monitor symptoms and vital signs
• assist with examinations and appointments
• coordinate care and communicate with physicians and community resources
• support quality assurance and infection control
• teach clients, families, and staff
• supervise less-skilled nursing personnel.
