What you'll do

• Assess patients and collaborate with the care team to develop and implement plans of care
• refer patients to other disciplines as indicated
• promote health and independence through teaching and rehabilitative measures
• supervise LPNs and home health aides
• complete accurate, timely documentation
• communicate patient progress to the clinical manager and care team
• plan and provide staff education
• perform assigned on-call responsibilities
• participate in clinical development and continuing education.
