What you'll do

• Ensure accurate dispensing, compounding, and distribution of medications
• verify prescriptions and counsel patients
• monitor medication outcomes and interactions
• provide medication therapy management and medication reconciliation
• conduct telehealth or in-person consultations
• collaborate on care plans
• identify medication regimen gaps and coordinate adjustments
• document encounters in electronic health records
• analyze medication data and participate in quality improvement.
