What you'll do

• Support referral sources with patient discharges to home infusion
• oversee home infusion care for patients with acute and chronic diseases
• coordinate referrals and registered nurse home visits
• assess patient and caregiver suitability for home infusion
• collaborate on nursing care plans with prescribers, field nurses, and the patient’s healthcare team
• maintain relationships with healthcare professionals
• stay current on therapies, industry trends, and accreditation standards.
