Systems Navigator (SN) — Fort Knox

What you'll do

• Conduct intake interviews and needs assessments
• provide referrals, follow-up, and advocacy
• coordinate with government agencies and civilian providers
• maintain service-provider information
• coordinate support groups or workshops monthly
• present EFMP briefings
• manage records under Army and DoD regulations
• facilitate warm handoffs
• collect data and complete monthly reports.

Why this role
