Extended Support Specialist - Direct Support Professional

The Extended Support Specialist role is onsite in Chicago, IL, with compensation of $18.16–$21.66 per hour. The position follows a six-days-on, seven-days-off rotation, with approximately 144 continuous onsite hours per rotation and scheduled sleep periods. UCP Seguin supports people with disabilities and special needs to participate in their communities and become productive, valued members of society. The specialist provides direct support to adults with developmental and other disabilities, helping them with daily living, personal care, community integration, and individual service plans. The role contributes to individuals’ safety, dignity, rights, well-being, and independence.

What you'll do

• Support personal care, daily living activities, transportation, medication assistance, community integration, household management, behavior support, documentation, and advocacy
• participate in interdisciplinary teams

• maintain accurate documentation
• help ensure individuals’ safety, dignity, rights, and well-being.

Why this role
