What you'll do

• Lead completion of Minimum Data Set assessments in compliance with federal and state regulations.
• Partner with the interdisciplinary team on individualized care plans.
• Ensure documentation and coding are accurate and aligned with reimbursement requirements.
• Review clinical data for trends and quality improvement, and stay current on regulatory updates and MDS practices.
