Mental Health Clinician - Social Worker II - Center for Gender Affirming Care

This position provides professional services to patients and families at the Center for Gender Affirming Care, which facilitates the use of individual, public and private resources so that optimal functioning can occur for both the patient and the family. These core services include assessment support needs and resource provision, mental health treatment and community resource planning. Mental Hea
