Case Manager, Apartment Treatment Program

The Case Manager role is based at the East Rockaway, NY office, with field services in Nassau County, NY; compensation is $21.44 per hour. South Shore Association dba SAIL’s Apartment Treatment Program supports residents in developing independent living skills. The case manager helps residents build independence and psychiatric stability through supportive counseling, skills training, individualized service planning, and coordination with treatment teams and community resources. The role also supports safe housing and continuity of care.

What you'll do

• Teach independent living, tenancy, housekeeping, socialization, vocational, educational, and community-integration skills
• coordinate admissions, transfers, and discharges
• provide weekly face-to-face field services and apartment meetings
• develop and update person-centered service plans
• provide supportive counseling, crisis intervention, case management, referrals, and linkages
• support medication self-administration education
• inspect apartments and coordinate approved repairs
• coordinate treatment-team activities, transportation, authorized resident funds, documentation, meetings, and training.

Why this role

Schedules include Tuesday–Saturday or Sunday–Thursday, with at least one evening shift per week ending at 6 or 7 p.m.; training is Monday–Friday, 9 a.m.–5 p.m.
