What you'll do

• Make 50 or more outbound calls daily to prospective patients and caregivers
• explain services and support
• guide patients through enrollment and surface goals and concerns
• respond to inbound communications and triage requests
• connect patients with appropriate team members
• obtain consent and complete enrollment surveys
• escalate urgent needs to clinical teams
• share feedback about patient and provider needs, referral trends, and access barriers
• work a structured schedule with coverage.
