WelbeHealth PACE provides comprehensive medical care and community-based services to help seniors remain in their homes and communities. The role supports participant health by conducting assessments and counseling, coordinating meal services, overseeing menus and nutrition staff, and ensuring meal delivery meets dietary and regulatory requirements.

What you'll do

• Conduct nutrition assessments
