What you'll do

• Coordinate care across clinics, hospitals, specialists, and community agencies
• oversee individualized care plans
• assess medical, behavioral, and social needs
• support access to care and appointment scheduling
• monitor treatment adherence
• coach members using motivational interviewing and trauma-informed approaches
• connect members with community resources
• coordinate transitions and discharge follow-up
• maintain documentation and productivity
• and follow Medi-Cal and HIPAA requirements. The role includes visits to member homes, hospitals, and community locations within the South Bay service area.
