What you'll do

• Provide skilled nursing treatments as ordered
• complete evaluations including vital signs and medication reviews
• coordinate with physicians, therapists, and other care-plan participants
• assess and chart client condition at each visit
• maintain accurate nursing documentation
• educate clients and families on home health procedures
• monitor and report symptoms or changes
• prepare patients for and assist with examinations or treatments
• modify treatment plans as indicated
• order, interpret, and evaluate diagnostic tests
• monitor diet and physical activity.
