The physician provides comprehensive primary care to patients of all ages, managing acute and chronic conditions and delivering preventive care. The role contributes to continuity of care through coordination with specialists and multidisciplinary teams, accurate EPIC documentation, and participation in quality improvement and value-based care initiatives.

What you'll do

• Provide outpatient primary care to patients of all ages
• diagnose and manage acute and chronic conditions
• conduct preventive visits, wellness examinations, and screenings
• coordinate care with specialists and ancillary services
• maintain clinical documentation in EPIC
• participate in quality improvement and value-based care initiatives
• collaborate with Advanced Practice Providers and multidisciplinary teams
• build long-term patient relationships.
