In-Home Services Certified Nursing Assistant (CNA)

The In-Home Services Certified Nursing Assistant (CNA) role is onsite at an InnovAge facility and/or participant homes in Pueblo, CO. Compensation is not specified. The role provides nursing assistant services to clients in their residences and supports participant care planning with multidisciplinary teams. The CNA provides routine, restorative and supportive care, monitors health indicators and reports changes to clinical supervisors. The work helps participants with daily living and care needs while documenting services and following established policies and regulations.

What you'll do

• Follow care plans and assist with daily living
• document care
• take vital signs and monthly weights
• monitor appetite, skin and functional changes
• assist with meals, mobility, positioning, transfers, skin care and other assigned support
• report unusual occurrences
• attend required in-services
• complete visit records
• perform assigned simple procedures and household services essential to home health care.

Why this role
