What you'll do

• Monitor and coordinate individuals’ health and follow-up care
• oversee medication administration procedures and inventories
• train, supervise, and evaluate direct support staff and LPNs
• maintain medical documentation and electronic health records
• collaborate with families, advocates, physicians, and team members
• respond to emergencies
• conduct clinical assessments
• coordinate post-discharge care
• provide medical coverage across assigned sites
• review RN on-call reports as needed.
