What you'll do

• Provide weekly clinical support
• assist with assessments, treatment planning, and progress monitoring
• coordinate with therapists, psychiatric providers, PRP staff, case managers, and community providers
• track referrals
• maintain timely electronic medical record documentation
• support audits and quality improvement
• serve as treatment team lead for assigned clients
• coach team members and help resolve service barriers.
